Listed below please find most insurance companies we participate with. Due to the ever-changing nature of the health industry, this list is not conclusive.

Since there are so many different insurance plans, it is the patient’s responsibility to understand their own policy and to know whether podiatry is a covered benefit.


As an enrollee of a managed care plan (HMO or PPO) that we are contracted with, you are required to pay the co-payment each time you are seen. Co-payments are due at the time of service. Please be aware, some insurance companies are starting to require additional co-pays for services such as x-rays.


In addition to the copayment, most insurance plans also have an annual deductible, as well as co-insurance. This is also your responsibility. The deductible is the amount of expenses that must be paid out of pocket before an insurer will pay any expenses. Coinsurance is a percentage of a provider’s charge that you may be required to pay after you’ve met the deductible this is usually about 20% of the allowable charge.


If you are enrolled in an HMO which requires a referral from your Primary Care Physician, you must have the referral with you in order to be seen by our doctors. If you arrive without a referral you have two options:

1. You can reschedule – with a $75.00 last minute cancellation fee

2. You can sign a waiver from your insurance company stating you understand there is no referral on file. If the referral is not received, you will be responsible for full payment of services.

No Insurance

A 25% courtesy discount is offered to patients without insurance that pay in full at the time of service.

We participate with Care Credit.

Collection policy:

In the event that there is a balance due from you after your insurance carrier has paid its portion, we will bill you. We ask that this balance be paid in full upon receipt of a billing statement from our office. A service charge will be added to all accounts over 60 days. Overdue accounts will be turned over to our collection service. To avoid this, please pay your bill promptly after you receive your first statement. If you do not understand the reason you owe a balance, please do not hesitate to contact our office. Our insurance specialist will explain the balance and answer any questions.
